Your doctor or healthcare organization believes you may benefit from additional support between office visits.

Through the Brilliant Care ACCESS Program, eligible Medicare patients receive ongoing help managing chronic health conditions such as high blood pressure, diabetes or prediabetes, weight concerns, high cholesterol, and kidney disease - all from home and at no cost to you.

Brilliant Care is one of a limited number of organizations chosen to participate in the Centers for Medicare & Medicaid Services (CMS) ACCESS Model, a Medicare innovation designed to improve care for patients with chronic conditions.

Brilliant Care brings experience in nurse-led care, remote monitoring, patient engagement, and technology-enabled chronic condition support to help patients stay healthier between doctor visits.

This is a Medicare-supported care program - not an insurance plan, not a sales program, and not a replacement for your doctor.

Before You Enroll

Brilliant Care provides these services as part of the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model, a federal payment model being tested by the Centers for Medicare & Medicaid Services (CMS).

Please note the following:

Information Sharing

To administer and evaluate the ACCESS Model, Brilliant Care may share certain health information with CMS. CMS may also access or request your health information, including through Health Information Exchanges or other authorized sources. Any sharing of information is subject to applicable federal privacy and security protections, including HIPAA.

Possible Control Group Assignment

As part of the ACCESS Model evaluation, CMS may randomly assign eligible patients to a control group. If assigned, you may be unable to enroll in the ACCESS Model with any participating provider for up to 12 months, unless otherwise specified by CMS. This will not affect your Medicare benefits, rights, or coverage.

Participation Information

If you choose to enroll in the Brilliant Care ACCESS Program:

  • Participation is completely voluntary.
  • Choosing not to participate will not affect your Medicare benefits or coverage.
  • There is no cost to eligible patients enrolled through Brilliant Care.
  • You may end your participation with Brilliant Care and, if you wish, enroll with another participating ACCESS provider beginning 90 days after your initial enrollment, consistent with CMS policies.
  • You may participate with only one ACCESS provider within a given clinical track at a time, although you may qualify for multiple ACCESS clinical tracks under CMS rules.
  • If permitted under CMS program rules, Medicare claims information may be shared with Brilliant Care to help coordinate your care and support the ACCESS Model.
  • All information is protected by applicable federal privacy and security laws.

Instead of waiting months between doctor visits, ACCESS helps patients receive regular support at home through nurse-led care, home monitoring, secure texting, and AI-enabled technology.
