We provide patients with user-friendly, connected devices for monitoring their health. Devices might include cellular-enabled blood pressure cuffs, glucometers, pulse oximeters, scales, or others depending on the patient’s condition. There’s no complicated setup – devices come preconfigured and ready to use. For example, our RPM blood pressure cuffs automatically transmit readings to our system with no action needed from the patient beyond applying the cuff.

Daily Data Capture

Patients take their readings at home as instructed Under Medicare guidelines, RPM typically involves collecting at least 16 days of data per month, but we often exceed that for closer tracking. The data flows electronically to our secure platform, in real time.

Real-Time Monitoring & Alerts

Our nursing team monitors incoming data. We set personalized thresholds and alerts – for instance, if a hypertensive patient’s BP goes above a certain level, or a heart failure patient’s weight increases overnight, our team is immediately notified. Customizable alerts and multi-condition monitoring, ensure nothing slips through the cracks. This allows us to intervene quickly: maybe a phone call to the patient to recheck a high reading, a medication adjustment in coordination with the provider, or even guidance to seek immediate care if readings are critically high.
