Each patient often interacts with one or two consistent nurses or care coordinators in our team. They build a relationship. So, when the patient calls or when we call them, it’s “Hi Ms. Smith, it’s Jane, how are you feeling today?” This personal touch means the nurse knows their story, their goals, their challenges. It fosters trust – patients open up about symptoms or admit when they haven’t taken meds, etc. Remote nursing programs benefit patients by offering easier access to care, assistance with SDOH needs, empowerment, and cost savings.Our nurses provide exactly that: they empower patients with knowledge, help solve non-medical issues, and are just there for them. For many patients, especially older adults or those living alone, our nurse might be the most regular health contact they have – almost like a health coach and advocate combined.
