What is ACCESS?
ACCESS is a 10-year national model from the CMS Innovation Center (CMMI), testing a new, outcome-based way to care for the chronic conditions that affect most people on Medicare. Brilliant Care is a participating ACCESS Partner Organization.
Connected home devices and an always-on care assistant keep your health numbers visible to your team between visits — not just once a year.
Under the model's Outcome-Aligned Payments, success is measured by real improvement in your condition — care is built around results, not the number of services billed.
