We can equip willing residents (with appropriate consent) with RPM devices – like blood pressure cuffs, glucometers, weighing scales or pulse oximeters – to keep an eye on their vitals and status daily. For example, if a resident has congestive heart failure, a sudden weight gain can signal fluid buildup – our remote scale will catch that and our nurse will intervene (calling the resident, coordinating with their doctor to adjust meds) before it becomes an acute crisis. As one study noted, home telemonitoring for blood pressure leads to better control versus usual care, which for seniors can prevent strokes or heart failure exacerbations. This approach helps seniors maintain stability.

On-Call Nurses for Residents

We can also host periodic health education sessions (virtually) for interested residents on topics like medication management, fall prevention, etc., boosting the facility’s wellness offerings.

Coordination with Facility Staff

We aren’t replacing any medical personnel the facility has; we supplement them. For ALFs with nursing, we work hand in hand – they might do the hands-on checks (like take a BP if our system flags something) and we provide guidance. For independent living (with maybe just a wellness director), we essentially become their remote wellness partner. We keep the facility management in the loop on general trends or urgent matters if needed. By being involved, we also free facility staff from some burdens. For instance, their staff might spend time on phone coordinating doctor appointments – we handle a lot of that. We can schedule appointments, manage prior authorizations, and follow up on behalf of residents, saving the facility staff and families a lot of legwork.
