An engaged patient is more likely to adhere to treatments, make healthy lifestyle changes, and alert us to issues early – all leading to superior outcomes. Frequent, Meaningful Contact

Engagement starts with regular contact that’s not just a checkbox call, but a meaningful interaction. We reach out consistently and always with a personal touch. By doing so, patients know someone cares about their progress. They look forward to our calls or messages rather than see them as a nuisance. This continuous relationship fosters trust and opens lines of communication. A continuity of care coordinator that a patient can rely on encourages them to share more and stay engaged in the process.

Personalized Goal Setting

Our care plans aren’t one-size-fits-all; we collaborate with patients to set goals that matter to them. Maybe a patient’s personal goal is “attend my granddaughter’s wedding in 6 months and be able to dance” – we use that as motivation for weight loss or exercise. By linking health actions to personal, meaningful goals, patients become much more invested in following through. Every achievement is celebrated (“Great job on lowering your A1c, I bet you’re feeling more energetic on your walks now!”). This positive reinforcement loop keeps engagement high.

Empathy and Listening

Our team practices motivational interviewing and empathetic listening. We let patients express their fears, preferences, and barriers. For instance, if a patient hates taking a certain medication due to side effects, we don’t just nag – we acknowledge their feelings and work with their provider to find alternatives. Patients often report that our nurses “really listen” to them. Feeling heard makes them more likely to voice concerns (leading to solutions) and to trust our guidance. Engagement is impossible without trust.

Education and Empowerment

Knowledge is power for patients. We take time to educate them about their conditions and the why behind recommendations. Patients in care management programs have reported happiness with the support and direct access they get, which helps them be more successful in their healthcare journey. Part of that is because they understand their health better. We might teach a patient with hypertension how to adjust diet and recognize sodium in food or help a patient with diabetes interpret what their glucose numbers mean day to day. We encourage questions. When patients understand, they engage more – they might start self-monitoring or calling us with questions rather than staying silent.

Technology for Engagement

We leverage patient-friendly technology. For those who are open to it, we have daily check-ins or symptom surveys. We send reminders via text, as well as micro learnings, or in some cases a daily or weekly joke or affirmation. For some patients, these tech touchpoints are fun and interactive, keeping them looped in even between calls. We always pair tech with the human element, which maximizes engagement – technology captures their data/attention, and our human team responds with personalized feedback.

Community and Family Involvement

We often involve family or caregivers in the engagement process (with patient consent). Teaching a spouse or child how to help the patient with exercises or diet, for example, extends engagement into the home environment. These broaden the patient’s support network and keep them engaged through multiple avenues.

We measure patient engagement every day. Engaged patients, as data suggests, tend to utilize preventive services more and manage conditions better.
