How we handle EHRs and data integration

We connect with most major EHR platforms (Epic, Cerner, Allscripts, NextGen, eClinicalWorks, Athena, etc.) through secure interfaces or APIs. For example, we have experience integrating with Athena such that our platform pushes RPM readings and care plan updates directly into the patient’s chart. This means when a provider opens the chart, they can see our notes, vitals, and summaries without logging into a separate system. It’s all in one place. Our technology partners and in-house IT team have developed methods of using middleware to ensure interoperability.

Depending on the EHR, our staff might chart directly in your system (many of our nurses are provisioned as remote users in the client’s EHR) or in our system that syncs over. In either case, providers don’t have to learn a new system to see what we’re doing. We document in a format that fits your normal chart structure. For instance, our monthly note will appear in the chart’s documents, or our telephone outreach will show up in encounter notes – however your EHR best accommodates that data. This integration prevents any “where do I find that info?” confusion.

We believe in one unified patient record. Some third-party programs keep their own separate records that don’t fully make it into the main chart. That’s not our approach. We want the data we collect (vital signs, patient conversations, care plans) to reside in the medical record, where it’s accessible and auditable. This also helps with things like meeting CMS billing requirements (we log time and notes needed for billing right in the chart, so if anyone checks, it’s all there).

With integration, your staff won’t have to re-enter our data or scan PDFs of our reports (a common problem when integration is lacking). For example, a blood pressure reading from our device can flow into the EHR vitals flowsheet directly.

We also integrate through communication channels – for instance, if we need a physician’s input, we might send an internal EHR message or task. That way, it pops up in their usual inbox (one they’re already checking). They can respond or sign off right there. This is far more efficient than separate emails or phone tags, and it keeps the communication linked to the patient chart. We keep everything “in the system,” which providers appreciate because it feels like we’re part of their usual routine, not an external entity.

By integrating with EHRs, we naturally operate under the same HIPAA protections and security frameworks you already use. We’re not introducing new data silos or potential security gaps. All data stays in a secure environment. We sign business associate agreements and ensure our processes comply with all privacy rules – treating your patient data with utmost care, just as you do.

When we update something like a care plan, we often notify via EHR note or flag, so the provider sees it at next visit. Conversely, if a provider changes a medication or plan during a visit, we see it in the EHR and adjust our management accordingly. This tight feedback loop is only possible through integration. The result: a well-coordinated care team with everyone literally on the same page (of the chart).
